New State Forms for Festivals

Many of the forms which were updated by the state and are needed for Festival and Fest Events are now available online. Forms that can be found on this site under the FORMS tab include:

• Medical Information & First Aid/Emergency Treatment Authorization Form
  (which now have cell phone number additions and Epi-Pen use) 
• Medical Administration Record Form, which must be submitted for each medication, and which also now includes changes regarding dispensing EVEN if the student is 18 years of age or older (*ALL medication must be administered by the festival nurse, regardless of the student’s age or Section 504 or Transition Plan)
• Special Dietary Needs Form
• Instrument Responsibility Form 
  

Please make sure that you are using the most current versions of our festival forms for the 2015-16 school year, particularly in the case of students advancing from district to region to state.  If the forms are not consistent, they will need to be resubmitted when advancing on so that all participants are in compliance.
