They have also added a Medication Administration Record form for students who require medication during the festivals. I apologize that this is last minute; we just got the information this morning. We would appreciate your help in getting the word out to the directors that there is a new form and an additional form that will require a physician's/licensed prescriber's signature for administering medications. This form need only be completed for students requiring medications. Please print out the attached forms (revised medical form and medication administration record form) on white paper and replace the original blue forms in the student folders for concert band, wind ensemble and orchestra. If you have already given the folders to the region host, please ask the region host to duplicate and distribute these forms to the students selected for All-State. You should leave the Special Dietary Needs form in the student folders (it is currently stapled to the old blue medical form). If you have any questions, please don't hesitate to contact me. Again, I apologize for the lateness of this request. The hosts of the two ensembles that already have the blue forms (Jazz and Vocal Jazz) will contact the directors of the students to give them the new forms. We will email the new forms to the directors of the chorus students as soon as the database is complete. Thank you for your help. -- Margaret S.
