Participating Director Festival Forms

These forms must be completed beginning at the district level; either during the pre-audition process or once a student has been selected for a district festival. There is a different online form for each type of festival, however once completed the form will follow the student should he/she advance. Please check with your district president to see when the form should be completed.

​FESTIVAL Forms to be completed by participants:
Forms that need to be filled out for EACH festival:
  • PMEA Principal Signature Page
  • PMEA Medication Administration Record**Only needed if student will be taking medication during the festival**
  • PMEA Housing Waiver & Release Form**Only needed if the student is opting out of a hotel stay**
Forms that can follow a student  **Unless there is a change in information** (from Districts to Regions and/or All-State, if applicable)
  • Student Medical Form
  • Student Dietary Needs Form (online) **Only needed if a student has special dietary needs/food allergies**
  • Instrument Responsibility Form (online)
  • PMEA Student Policy Acceptance Form
  • PMEA-Policy Statement Male-Altos-Sopranos ; PMEA-Policy-Statement-Female-Tenors-Basses
  • Student Information Forms will be sent to participating directors directly from the Host.

FEST Forms to be completed by participants:
  • Student Medical Form
  • Student Dietary Needs Form (online) **Only needed if a student has special dietary needs/food allergies**
