PMEA & District 1 Forms

The Executive Board approved revised forms beginning with the 2008-09
school year.
Additional fields have been added to the forms to gather more information that
is helpful to festival hosts and selection committee members. Several
revisions were also made to policy statements on the reverse side of the Student
Application forms to provide clarity to directors, parents, and students.
In addition to the PDF versions below, a new feature of these updated
forms are versions set up with enterable MS Word fields so that information can
be typed directly into the file and printed out. Follow the directions
below to save the file to your desktop so that you can complete the form in
Word.

District 1 Scholarship Application
PMEA District 1 Scholarship
The Scholarship Committee has developed the following criteria
for awarding three music scholarships to students in our
district. Each award will be $1,000.00 (approved
August 2008)
1. The recipient must have definite plans for a career in music
education or music-related field.
2. Three scholarships will be awarded each year; one in strings,
one
in wind/percussion and one in vocal music.
3. The award will be based on music achievement and academic
excellence.
4. An applicant must currently be a senior in a District 1 High
School
and a participant in the current District 1 Festival. A
student’s
placement in the festival is not a factor in granting the
award.
5. Two recommendations must accompany each application. One
recommendation must be written by the student’s home school
Music Director PMEA member, and one must be written from any
of the following: private music teacher, high school teacher
in
any discipline, guidance counselor, church music director, or
high
school principal.
6. The student must include with the application and
recommendations, the following: a) a high school transcript,
b) an
essay on why music education/music is important, why the
student wants to pursue a career in music, and future
ambitions
and goals upon graduating from high school and college.
7. Completed applications must be mailed to the Scholarship
Committee Chair by April 1, 2010 to:

8. The award recipients for the District 1 Scholarships will be
announced at the recipients’ spring concert. A PMEA officer
will
present the award.
9. Applications will be available in the fall newsletter and at
Honors
and District 1 SHS Festivals.
10. The committee reserves the right to give no award in any
area
where applicants are not deemed qualified.
